During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. We accept most insurances.
Patient Forms
Please print and fill out the New Patient forms so we can expedite your first visit.
Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
- Patient's insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about patient's medical and surgical history
- Recent test results, x-rays, or relevant records
Payments & Payment Plans
We accept Cash, Checks, Debit, MasterCard, Visa, Discovery, and AmericanExpress
We now accept pre-payments for surgeries that need to be done in a hospital setting, and promise to pay plans on all INSURANCE PATIENT RESPONSIBILITY. We also can automatically bill your credit card (on file) so you do not have to worry about keeping up with medical insurance EOB -ask us about these exciting new programs.
Insurance Plans – Referrals – Authorizations
Our contracts with insurance carriers are subject to change. Check with your carrier to make sure we are listed as a provider. In some instances, a referral may be needed from your primary care provider. If that is the case, it is the patients responsibility to obtain the referral. If you come to our office without a referral and are seen by the doctor, you will be responsible for the service.
All insurance plans are NOT created equal, your plan may require a copay to see a specialist, a deductible to be paid at the beginning of each policy year for office visit or for out patient surgeries, you might even have a different one for DME products. You might have a co-insurance plan but not a secondary insurance policy. There might be an exclusion in your policy that states you can not see a specialist or you can only see a specialist a few times per year. All of theses are taken into consideration when your insurance company tells us what we are to collect at time services are rendered. It is impossible to know, 100%, what your cost will be. After the electronic claims have been sent, your insurance company will send you the Explanation Of Benefits or EOB about 7-10 days before we receive it. Only after that, will we know the final cost of each visit. A statement will be sent to you for the remaining amount due to Highlands Foot & Ankle, LLC.
Some of the insurance plans we accept are: Ambetter, AmericaGroup, Blue Cross Blue Shield, Care N Care, Cigna, GEHA, Humana, Medicaid, Medicare (Palmetto GBA), Molina, MultiPlan/PHCS, Scott & White, Superior Health Plans, CHIP, Tricare, UMR, United Health Care, and many other HMO, PPO and POS plans on the way! We also accept other insurance plans not listed, with out of network coverage, so please contact your insurance health plan & ask if you have out-of-network coverage.
